Physical Therapist

 Our Physical Therapy Team, like the home health nurse, develops individual care plans for their home health patients. They assess the home for safety issues and develop a home exercise program based on the patients home environment and the disease processes of their patient.  The following, are the typical types of services the physical therapy team provides for their patients:

• Home safety evaluations/instruction/recommendations
• Ambulation training
• Transfer training
• Energy conservation techniques
• Fracture management
• Walker/cane/wheelchair training
• Dressing/undressing training
• Balance/strengthening instruction
• Post-Operative care, range of motion, and weight-bearing safety measures

 Our Occupational Therapy Team is a specialized group of therapists that understand the importance of identifying home safety issues and promoting independence for their patients, while performing their activities of daily living such as dressing, undressing, bathing and grooming.
The following are the typical types of services the occupation therapy team provides for their patients:   

• Home Safety Evaluation/Instructions
• Home modification recommendations
• Individualized home exercise program
• Assistance/training with Dressing/Undressing
• Assistance/training and reinforcement for eating and meal setup
• Assistance/training with showering/bathing
• Cognitive training for moderate to severe impairment
• Hand therapy
• Energy conservation techniques
• Cardiac rehab

 Our Speech Therapy team focuses on the patients' breathing, swallowing, speaking, cognitive skills and communication. The following are the typical type of services the speech therapy team provides for their patients:  

• Development of memory tasks, problem solving and reasoning projects
• Swallow/ eating evaluations and training
• Memory/cognition evaluations and training
• Communication training for language disorders
• Develop verbal or nonverbal systems to improve communication
• Develop visual, auditory aids, and can calendars to assist with early cognitive impairment
• Help modify patients' diets and instructions for safe swallow strategies for patients who are manifesting swallowing difficulties.
