Medical Social Workers
Our Medical Social Workers help our patients address their social, financial and psychological aspects of recovery. Many times finding the right community resources can make all the difference in the outcome of our patients progress. The typical types of services our Medical Social Workers provide to our patients and their family members are the following:
- Provide counseling
- Provide community resource referrals
- Assisting the patient and their families with advance care planning, health directives
- Assistance with access to financial assistance
- Assistance with long term planning
- Assistance with transportation to appointments
Community Liaisons
Our Community Liaisons (commonly referred to as our sales team) manage the communication and referral processes between the home health agency and the community health care providers, such as hospitals, physician's offices, wound care centers, assisted living facilities, or skilled nursing facilities.
