Education and teaching on the  definitions, symptoms, risks and management of congestive heart failure (CHF), Chronic obstructive pulmonary disease (COPD), hypertension (HTN) and all other heart, lung and vascular diseases.

 Our experienced home health nurses will be providing wound care according to  the patient's specific needs and the physician's orders. They will be providing teaching and education for the patient and the caregiver regarding diet, nutrition, signs and symptoms of infection, proper wound care/managment and the wound healing processes. 

 Education and training on the disease processes associated with your pain, including pain relief techniques,  nutrition, exercise programs and medication management to reduce the intensity, frequency, duration and your overall pain level.

 Education and teaching on the definition of diabetes, types of diabetes, how to use your blood sugar glucometers, how to give insulin injections, signs and symptoms of hyperglycemia and hypoglycemia, management of diabetes through diet, medication, exercise and daily foot/skin inspections.   

 Education and teaching on the definition and types of Alzheimer's disease, the risk factors, signs and symptoms, and how to better manage the disease process with medication and memory/cognition exercises, and caregiver education and support to prepare the caregivers for the disease progression.  

Education and teaching on the definition and causes of urinary incontinence, the different types of urinary incontinence, the risk factors associated with urinary incontinence, bladder irritants and the treatment and management of urinary incontinence through medication, exercises, nutrition and bladder equipment required

 Our care transition program simply assures that your  transition from the hospital or rehab facility and back to your home is  timely and seamless, and that you will receive the care that you need in the safety and comfort of your own home.
