Home health nurses

  Our Home Health Nurses are the backbone of home health. They have many years of nursing and home health experience. They understand that every patient is unique, and the care plan developed for each patient must be designed uniquely and according to the needs and the diagnoses of each patient. Our home health nursing team consists of Registered Nurses (RN's) and Licensed Practical Nurses (LPN's). The type of services the home health nurses would be performing could be any of the following for the patient and or caregivers:  

  • Disease Management Teaching
  • Medication Education, Teaching and Management
  • Infusion Therapy
  • Wound Care
  • Medication Reconciliation
  • Respiratory disease management
  • Foley Catheter Management
  • Assisting and Monitoring Pain Management
  • Post-Operative Care

Psychiatric Nurses

 Our Psychiatric Nurses receive special training and education on how to recognize and treat the signs and symptoms of depression, anxiety, panic disorders, and other psychiatric illnesses our patients may be experiencing. We know that our patients' healing and recovering process does not just include the physical aspects of health, but also the psychological aspects.
The type of services the Psych Nurse may be providing our patients are:

  • Monitoring and assisting with medication management
  • Monitoring and education for signs and symptoms of psychiatric disorders
  • Implementation of mental health treatments to coincide with the physical medical health issues   
  • Instructing patients on methods to manage unsafe behaviors
  • Assess and instruct patient/caregiver to improve coping skills when in crisis
